Arise Home 4 Care (AH4C) is guided by a person-centered, strengths-based philosophy of care that recognizes the unique abilities, needs, and aspirations of each individual. We serve adults who have developmental disabilities and mental health diagnoses. We are committed to providing a safe, supportive, and nurturing environment that promotes dignity, respect, and personal growth. We currently have a home in the City of Milton with 4 bedrooms available to house those individuals who need a home.

At AH4C our approach emphasizes individualized support, recognizing that each individual develops at different paces and requires varying levels of assistance. Care is delivered in a manner that encourages independence, self-advocacy, and life skills development, while ensuring appropriate supervision and guidance.We at AH4C believe that positive relationships are central to effective care. Staff foster trusting, respectful, and supportive relationships with residents, families, and guardians, creating a sense of belonging and emotional security. Open communication and collaboration are encouraged to support informed decision-making and continuity of care.

Arise Home 4 Care promotes holistic well-being by addressing physical, emotional, social, and developmental needs. Services are delivered through structured routines, meaningful activities, and opportunities for education, vocational training, recreation, and community participation. We are committed to inclusion, cultural sensitivity, and non-discriminatory practices. All residents are treated with fairness and respect, and care is provided in accordance with safeguarding principles, ethical standards, and regulatory requirements. Through this philosophy of care, AH4C supports each individual in building confidence, resilience, and the skills necessary for a successful transition toward greater independence and community integration.

In addition to these essential services, our operational infrastructure includes ongoing facility maintenance, robust staff training programs, and established channels for emergency support and crisis intervention. To assist with crisis intervention, we haveutilized the services of 2 registered Behavioral Analysts. Coordination with external
service providers ensures prompt response to facility needs and continuity of care, while regular reviews of systems and processes uphold best practices and compliance with evolving regulations.

This integrated approach allows AH4C to create a safe, welcoming, and adaptive environment where both residents and staff are supported in achieving positive outcomes and long-term stability. Informational and educational resources, including mission and vision statements, program descriptions, frequently asked questions, and policy summaries, will be prepared to support transparency and informed decision-making for families and guardians. Welcome and orientation materials will also be provided to assist new residents and families in understanding daily routines, available supports, and the resident
rights.
